Observation

This licensor was unable to review staff persons Sousan Behshad and Sara Valizadeh's staff file during the virtual visit. The provider stated that these documents were on her computer and/or in her emails. This licensor instructed the provider to email the staff files be 4pm on 3/30. As of 4:30pm on 3/30 the provider had not emailed these documents. This licensor is unable to confirm that the staff persons have the obtained the required trainings. It was observed that staff person Sara Valizadeh had started work 3/01/2022 and had not submitted a background check.
